Weeneebayko Area Health Authority (WAHA) is seeking experience candidate to fill the key role of Specialty Clinic Patient Care Coordinator.

What We Do

We are an integrated health system providing the full continuum of care to the six communities of the western James Bay coast with over 500 employees, physicians, and partners in care. Position Profile Requirements:
- Effective interpersonal relationship, communication, and organizational skills
- Dependable, reliable
- Sensitive to cultural needs of the client
- Adaptable to changes in priority and workload
Key Responsibilities:
- Coordinating the Specialty Clinics
- Planning and coordinating Specialists’ travel and accommodation with their clinic, IPS, NIHB, Physician Service Coordinator and Family Medicine Clinic
- Ensure accuracy of client demographics
- Ensure client confidentiality and privacy is always maintained
- Prepare for clinics by ensuring that charts are adequately stocked, and that all pertinent information is readily available
- Maintain records of all referrals and complete monthly reports. Track and file test results.
- Make appointments for patients with specialists
- Carry out clerical duties
- Advocate for and communicate client needs to appropriate health care professionals. Interact with outside agencies and social services to facilitate client services for on-going/ continued care
Qualifications/Experience:
- Diploma in Nursing required, Bachelor of Science in Nursing Preferred
- Registered with College of Nurses of Ontario and in good standing
- Certification in BLS and other mandatory in-services prescribed by Weeneebayko Area Health Authority
- Knowledge of the organizational structure and health system in the region

Salary: $89,797.50 - $128,700.00

Compensation
$46.05 - $66.00 per hour
